The role:

The Integrated Health Services (IHS) Program Coordinator oversees all functions of the program and ensures that IHS staff are providing or arranging for the provision of services necessary to enable clients to achieve optimal physical and behavioral health. The IHS Coordinator will ensure that IHS staff are assisting clients in identifying, securing and sustaining resources necessary to live a healthy lifestyle and to recover fully from behavioral health problems. The IHS Coordinator will be responsible for assessing and collecting outcomes from the coordination of these services.

The IHS Coordinator acts as a liaison between community medical practitioners, Federally Qualified Healthcare Center, local hospitals, schools, clinical staff, family members and others on behalf of the client ensuring client needs are met in a consumer friendly and timely manner. The primary role of this position is one of networking and developing relationships with consumers as well as referral sources. Additionally, troubleshooting on behalf of the client when services are delayed or when client needs immediate assistance or is experiencing a behavioral health crisis.

What you will need:

A Bachelor’s degree in social work or related human services field is required. At least two years of experience must include supervising staff.

Five years of experience in the delivery of mental health, substance use or co-occurring treatment services to children and adolescents in a public or private agency is required

Experience in a complex care setting including client coordination in a multidisciplinary environment, with a focus on facilitating integrated approaches to community health and well-being is preferred

Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.

What you will do:

Direct Service Responsibilities:

Assess current mental health, substance use and other multi-occurring and complex needs as applies to each client.

Identify, communicate and collaborate with medical providers, behavioral health providers and other community support providers to facilitate care and integrate resources to assure the client behavioral health needs are being met.

Assess for informal and formal support systems.

Facilitate individualized care planning and mutual goal setting.

Educate clients and their families or care advocates in wellness, medication and behavioral health services.

Participate in ongoing quality improvement efforts and monitor/evaluate service outcomes.

Complete required written reports and documentation according to policy.

Ensure that crisis plans are developed, as necessary.

Coordinate community based services that involve linkages with referral sources.

Coordinate transportation for clients to appointments when necessary.

Coordinate and be familiar with all aspects of outpatient funding, including understanding of EMR and funding coordination internally as well as credentialing and compliance with third party payers.

Meet regularly with stakeholders throughout the designated region ensuring seamless transition and referrals to Treatment Service programs.

Ensure positive relationships with referral sources and address concerns directly in a timely manner.

Oversee the scheduling and coordination of initial, crisis, intake and other appointments for clients.

Ensure that all required client signatures and insurance verifications are obtained upon intake.

In the absence of a counselor, provide consultation, information and referral services to clients, completing an intake or initial assessment if necessary.

Route on-line inquiries to the appropriate location or service provider.

Provide information to community service agencies or to groups when requested.

Participate in special projects as assigned


Supervisory Responsibilities:

Attends staff meetings, training sessions, and program meetings.

Maintains a good working relationship with other staff members.

Meets for supervision and prepares materials to be discussed before supervisory sessions so that maximum benefits can be derived from supervision.

Establishes positive relationships and networks with community partners on a routine basis.

Provides community presentations on relevant topics and is accessible to the media.

Hires, trains, supervises, and provides required evaluation and consultation to all IHS staff.

Reviews and approves as designated required documentation for counselors and/or other staff.

Maintain file integrity meeting accreditations and licensure standards.
